Carers

Do you voluntarily look after someone who is ill, frail or has a chronic condition, a physical disability, a learning difficulty, a mental health problem or substance misuse? If so, you are are a Carer and we would like to support you. 

The Practice is proactive in identifying voluntary Carers who are registered at with the Practice especially those people who may be caring without help or support.  If you are a voluntary Carer, please ask at Reception for a Carer's  Referral Form which you can complete to let us know about your caring responsibilities.  
This information will be added to your computer record so that all clinicians at the Practice who are in contact with you are aware that you are a Carer. 
 

Carers support

Online Carers Group  - the group meet online normally the first Thursday of each month, to share information, advice & offer each other support.
