New Patient Registrations 

The practice accepts new patients who live within the Practice area , please ask at reception for more details.

Registration forms and new patient questionnaires are available at reception.

All new patients are welcome to make a New Patient Health Check appointment, this is essential if you have a chronic condition or are on medication that requires monitoring.   This ensures that all reviews, blood pressure readings or blood tests are up to date. 

Please complete the new patient questionnaire with as much information a possible.
It can take many months for us to receive your medical records from your previous GP Surgery.
If you are transferring from a Practice in England we often receive incomplete records. Thank you. 

Changes to personal information

 

If you change your name, address or telephone number please inform us as soon as possible so that your records can be updated.
It is your responsibility to inform hospitals if any of your personal information changes.

If you move out of the Practice area you will be advised to register with an alternative GP.

Please note that official documentation will be required to confirm changes of name or gender  -e.g. marriage certificate, deed poll etc.

The Practice aims to provide the very best care and treatment we can. It is important that we hear patient's feedback regards our services and care.  

The Practice Concerns Procedure ensures that when a concern is raised, it can be investigated and resolved as quickly as possible.

What to do if you have a concern

Often the concern can be dealt with by speaking to a member of the Practice Team.

  • You can raise the matter with a member of staff who will try to resolve it immediately 
  • You can ask to speak to  Mrs Julie Evans, Practice Manager or Francesca Phillips Assistant Practice Manager on  01239 820397 
  • If you prefer to put your concern in writing, please include as much information as possible, address your letter to Mrs Julie Evans Practice Manager.
     

What the Practice will do.

  • If the concern cannot be resolved immediately the Practice will acknowledge your concern in writing within two working days. 
  • Details of the concern will be shared with the Partners at the Practice at the earliest opportunity.
  • If the concern relates to someone other than yourself, written consent from the patient will be required before the Practice can fully respond.
  • To investigate the concern, the Practice may need to access your medical records, if you do not wish this please inform the Practice Manager in writing. 
  • The concern will be investigated and we will aim to provide you with a full response within 30 working days.  If for any reason we are unable to meet this timescale, we will write to keep you informed. 
  • The Practice will provide a response which includes an explanation, an apology if appropriate, and what action the Practice is taking or has taken to help prevent any similar problems in the future.

Llais  (formerly known as Community Health Council )

Llais provides a FREE and independent advocacy service which is able to help patients or the people acting for them to raise a concern. 
Llais officers can give advice and provide support during the procedure.

Preseli Practice is an approved Training Practice. We provide training placements placements for junior qualified doctors and medical Students 

The Practice feels it is vital to support the future generations of doctors in gaining experience of working in a rural GP Practice. 
 

GP  Registrars are fully qualified doctors who are training in General Practice and are normally with the Practice from 6 to 12 months. 
Current Registrar details August 2024 Dr Bethan Green  and Dr Lucy Chibueze

 

Foundation Year One Doctors  are fully qualified doctors who are with the Practice to gain experience of Primary Care alongside working within Secondary Care Settings.  Our current Foundation Year one doctor is Dr Norhan Shaykhon 

Foundation Year Two Doctors are fully qualified doctors who are with the Practice for 4 months to experience working in Primary Care.
August 2024 - there are no Foundation Year two doctors on placement 


After a short period of induction all Doctors start seeing patients in their own surgeries across both our sites.
They are supervised by the Practice GPs working that day.

University Medical Students  

The Practice hosts final year medical students from both Cardiff and Swansea Medical Schools, this can be from  a period of one week to two months.
This is excellent opportunity for medical students to experience Primary Care first hand. Medical Students may be sitting in with the clinician you are consulting with and may also be the first person you see for your appointment.
