⭐ Carers & Young Carers Support

If you look after someone who is ill, frail, disabled, has a chronic condition, learning difficulty, mental health problem or substance misuse, you are a Carer — and we would like to support you.

The Practice is proactive in identifying Carers, especially those caring without help or support. If you are a Carer, please ask Reception for a Carer’s Referral Form

Why it’s important to tell us you’re a Carer When we know you are a Carer, we can:

  • consider your caring responsibilities when planning your care
  • provide Carer‑specific support and signposting
  • ensure clinicians are aware of your situation

Carer’s Needs Assessment
You are entitled to a Carer’s Needs Assessment from your local council.
This looks at:

  • your caring role
  • how caring affects your wellbeing
  • what support you may need
  • respite options
  • financial or practical help

You can request an assessment directly from your local authority

Young Carers are children and young people who help care for a family member. Support is available locally to help with school, college, wellbeing, activities, and respite.
